Great article.
Unfortunately, people frequently don't understand how to effectively collaborate, and have bad experiences that make them reluctant to try. Consensus paralysis is just one issue.
There are practical and simple skills and tools that people can learn that make collaboration easier and more successful.
Also, I don't know that Carly Fiorina is a good example of a collaborative leader. I don't have personal experience but I have read she was fairly autocratic.
